facilitation of research on improving support and education The number of older and chronically ill adults will increase markedly in the coming two decades. Growth in the number of older adults and continued improvement in medical care will allow more individuals to live longer with chronic disease. In an attempt to prepare for the health care of these older adults, researchers have begun generating new models for health care that promise to improve efficiency and outcomes in chronic illness care.
